  • DPC is a model of health care that prioritizes the doctor-patient relationship and eliminates the middle men of corporate or fee-for-service medicine that is a staple of the traditional healthcare system. We aim to operate our clinic in a way that keeps healthcare affordable for patients, as well as accessible so you can reach your doctor directly and in a timely fashion.

  • Fortis DPC is a direct primary care clinic founded with the mission to provide affordable, accessible, comprehensive primary care services. We aim to prevent chronic disease and its consequences, and when chronic disease already exists, we aim to not only treat it but potentially reverse it.

    Traditional healthcare has become too reactive, and we believe in being proactive. The DPC model allows you and your doctor to communicate directly, more often, and more effectively to ensure you are getting the best care possible. Dr. Scoles is a board-certified family doctor with a strong passion for lifestyle medicine. Rather than simply finding problems and medicating them, we will spend time exploring your nutrition, exercise, and sleep habits and how they impact your wellbeing. We won’t just tell you what to do - we will educate you and empower you to achieve your highest wellness potential.

  • A monthly membership at Fortis allows you to see your doctor for routine visits (chronic or preventative care) or when things come up unexpectedly (acute care). There are no excessive wait times to get in. Your appointments and communications with the doctor are built into the membership price, rather than billed individually (no copays). We offer in-person as well as virtual appointment options when appropriate. Certain procedures are included in the membership as well. Additionally, you receive access to significantly discounted lab work and diagnostic tests (imaging, etc) as well as discounted medications dispensed in the clinic.

    A membership agreement is reviewed prior to any payments, so you will have ample opportunity to ask questions and ensure it’s a good fit for you. There are no long-term contracts required with our membership. The membership is billed monthly.

  • We do not bill your health insurance for our services. DPC involves a transaction between the doctor and the patient. You should still have health insurance for catastrophic events. Many people who use DPC carry high deductible plans. The cost of your membership at Fortis is far less than the average insurance deductible.

  • We see patients with all types of insurance, but we do not bill insurance. Dr. Scoles is “opted out” of Medicare and Medicaid. She can see you as a patient if you have either of those, but she cannot bill your insurance.

    We also cannot see you if you carry a Medicare Advantage plan, as these plans are highly restrictive.

  • You cannot use FSA/HSA funds to pay for your membership at Fortis. However, you can use these funds to pay for lab tests, imaging studies, or medications.

  • The idea behind our practice model and our mission is to get back to the roots of primary care, where we can be there on a reactive basis when things come up unexpectedly, but also work with you proactively to prevent negative health outcomes. Preventative care provides a strong foundation for your wellbeing. In the DPC model, we have the time to offer you personalized care. So if you are generally healthy and you carry a high deductible health insurance plan, having a DPC membership can be crucial to maintain your health and have convenient access when something does come up in your health.

  • Yes. DPC is not health insurance. It is recommended to maintain health insurance for catastrophic events, similar to the way you maintain car insurance in case you get into an accident.

  • Having a membership with Fortis DPC entitles you to access to your doctor. Once you sign up, you will be provided with a phone number to use for urgent needs. You will also be able to text or email the doctor for non-urgent needs. These communications go directly to Dr. Scoles, rather than a triage nurse or other type of non-clinical administrative worker.

  • Dr. Scoles may order lab tests. Some of these blood or urine tests can be collected in the clinic during your appointment. In-house labs are available at a significantly discounted price. You have the option of taking the lab orders elsewhere to be done, especially if you would like them billed to your insurance carrier.

  • We do not perform imaging services in our clinic, but we can order these tests. They can be done at any facility that offers the service. You can either run these charges through your insurance plan or you can utilize a local independent imaging center where we have negotiated affordable cash prices.

  • We try to prevent hospitalizations and minimize the need for referrals, but Dr. Scoles can order a referral to a specialist if this is needed. If you already have a specialist, Dr. Scoles can communicate with them about your care. The same goes for hospitalizations.

  • For an individual adult, our monthly membership is $99 and there is a one-time enrollment fee of $99. The cost is billed automatically each month, similar to the other subscription services you likely already carry (phone bill, streaming services, etc.). We understand that life happens and things come up, so you can cancel your membership with notice, and this is outlined in our membership agreement. Click here for more details on our prices.

  • Dr. Scoles is a board certified family physician who provides comprehensive primary care services. This includes, but is not limited to preventative care, diagnosis and treatment of medical conditions, in office procedures. See a more detailed list here.   • No. We highly encourage a membership, but we can see you without a membership for a one-time visit at a flat rate of $199. Restrictions apply. Submit our form for more information about this. You can also call 715-204-9279.

  • You can get started with a short “meet and greet” appointment with Dr. Scoles. This allows us to get to know each other a bit, learn about the clinic, and answer questions you may have. This can be done over the phone or by video appointment. Click here to get a time booked to chat.
